UD has a protocol for occupational injuries/incidents and near misses that begins with a first report. Near misses are reported the same as occupational incidents. Reporting parties are asked to take initial appropriate actions and perform a root cause analysis alongside EHS.

A recent example was a fall that occurred from a work vehicle. The employee was attempting to enter a large pickup truck when the bolts on the grab bar had broken away from their mounting. The employee did not require medical attention, but the root cause analysis has prompted Transportation Services to investigate similar models within the UD fleet and to install a step on each involved vehicle to assist with entry.

Remember to report all occupational incidents and near misses to help keep each other safe. The First Report of Occupational Injury or Disease form is found on the UD EHS website, on their Forms page under the Injury/Illness section.
